We have openings for case management for healthcare. Options are abundant for you! Remote positions for South Carolina residents. Current openings in Ridgeland, Anderson, and Rock Hill areas. Candidate will reside in the area of work. While Case Manager is able to set their own schedule and work from home, Case Manager does home visits and are required to attend once a month in person meetings.

Responsibilities: 
Be responsible for assessing clients’ treatment needs and developing, monitoring, and evaluating treatment plans. Coordinates services, arranges resources like transportation, and ensures clients receive necessary care. Monitors cases by verifying attendance, evaluating treatments, advocating for services, intervening in crises, and providing personal support.

Details:

• Full-time and part-time positions are available working in the county of your choice.
• Set your own work schedule between the hours of 8 am – 7 pm.
• Pay: $52.50 per case.
• Candidates must have a BS in Social Work or Human Services related field along with 2 years experience with assessments.
